Our Commitment to You
We reserve your appointment time exclusively for you. Because psychiatric appointments require preparation and clinical continuity, late cancellations and no-shows affect both your care and our ability to serve other patients in need.
Cancellation Policy
We require at least 48 hours' notice to cancel or reschedule any appointment.
48+ hours in advance — No charge
24–47 hours in advance — $75 cancellation fee
Less than 24 hours — $100 cancellation fee
No-show (no contact) — Full session fee charged
Cancellation fees are not billable to insurance and are the patient's responsibility.
Intake Form Completion Requirement:
All intake forms must be completed at least 48 hours before your appointment to allow time for review and help me make the most of our time together. If your forms are not completed by this deadline, your appointment will need to be rescheduled.
After reviewing your completed forms, I may send additional questionnaires or scales related to the reason for your visit. These are relevant to your care and will help me prepare for our appointment. The 48-hour completion deadline does not apply to additional questionnaires or scales sent after I have reviewed your initial intake forms.
No-Show Policy
A no-show is defined as failing to attend a scheduled appointment without prior notice. After two consecutive no-shows, Mind Matters reserves the right to discharge the patient from the practice.
Late Arrivals
Appointments begin and end at their scheduled time. Arrivals more than 15 minutes late may be rescheduled and subject to a late cancellation fee.

Voicemail and email are monitored during business hours. Please do not use these channels for urgent clinical matters.
Form Requests:
Completion of forms requested by patients (such as disability, FMLA, school, employer, or insurance forms) is billed at $75 per hour of time spent. Time is billed in 15-minute increments. Charges are billed to the credit card on file. Before any work begins, you will receive a good faith estimate of the time I anticipate spending on your forms. Work on your forms will begin once you have approved the estimate.
Please allow up to two weeks for forms to be completed. For expedited forms with a 48-hour turnaround, an additional $100 fee applies.
Form completion fees are not billable to insurance and are the patient’s responsibility.
